• Visa Application from in quadruplicate
  • Photocopies of data page of the applicant passport
  • Photocopy of the Marriage Certificate or Marriage contract (if dependents are included in the application)
  • Two photographs (3.5x 4.5 cm) of applicant and of each dependant.
  • Medical Report of the main applicant.
  • Documentary evidence of remittance USD 250,000 to Sri Lanka
  • A Police Report obtained from his country of domicile.
  • Details of the project in which the investment is to be made should be provided.( BOI Agreement, BOI Certificate, Memorandum and Articles of Association, BOI Recommendation, Line Ministry Recommendation) should be provided. If the Project involves an existing company, the relevant registration documents is required
